The merits of self body's cartilage – an introductory comparison to artificial materials' rhinoplasty

Due to the atmosphere of orthopedic surgery in recent years, rhinoplasty surgery is no longer a strange thing to us. With the two-step (Korean style) rhinoplasty surgery, it is not an unreachable dream to have a perfect nose. However, in addition to the safety and effect of the surgery, it is worth of discussion from different aspects on the issue of how to choose the materials of the implants for the duration matters.

So far, the two materials that can be used as long-term rhinoplasty fillings are silicon gel nasal (artificial) bone and Goretex nasal bone which requires further elaborating. These kinds of materials are easy to obtain, and the effect are stable. Along with their highly plastic feature, they are widely accepted by orthopedic doctors and patients. However, it is still inevitable for the artificial materials to encounter problems such as affection, rejection, material exposure or even penetration from time to time. These factors enable people who are considering of having rhinoplasty to give it a second thought or even to choose the alternative solution to use their own cartilage to sculpture the whole nasal shape. This kind of surgery needs a huge amount of cartilage. The most suitable part in one's body to find this much cartilage would be the ribs of the front chest. This is the theme of this passage – rhinoplasty surgery with self body's cartilage.

Rhinoplasty surgery self body's cartilage was first utilized on the reconstruction of nasal injury and congenital nasal defect. In the recent ten years, there has been some orthopedic starting to use this method onto rhinoplasty. Rigidly speaking, this still belongs to the domain of nasal reconstruction instead of rhinoplasty, so it may be a little bit strange to many people. As a matter of fact, the process of self body's cartilage rhinoplasty is not that different from the Korean (open) rhinoplasty with traditional artificial materials. The biggest difference falls on that the doctor would have to remove a piece of rib cartilage from the patient's front chest and sculpture it into nose bridge and apex to replace the artificial materials and implant them into the patient's nose. The total time of the surgery would take about three hours with the general anesthesia of the patient. Rhinoplasty of this kind has the following advantages:
1. Low affection rate: Because the implants are all of the patient's body parts, this kind of surgery seldom cause affection or inflammation.
2. No rejection phenomenon: This kind of surgery completely avoid the rejection problem that artificial materials may have.
3. No allotrylic implants: Patients would not have to worry about any kinds of collisions after the surgery.

However, rib cartilage rhinoplasty still has the disadvantages below that are not yet overcome:
1: Cartilage absorption rate: Implanted cartilage will be absorbed by the body from 10% to 20%. This makes it difficult to predict post-surgical effect.
2. Unpredictable arcuation: Because of the memory effect of the cartilage cells, the implant would be easily bent to its accustomed side. The deflection of the nasal bridge can be seen from the surface. This problem may occur several years after surgery, there is no way to prevent it.
3. Hard to maintenance: The cartilage will fuse closely with the skin tissue. It would take an open surgery in the future for further maintenance if required.
4. The scar on the front chest: There will be a 4cm scar from where the cartilage is get left on the front chest. This will influence the perfection of the skin.
5. The risk of general anesthesia.

From the analysis mentioned above, we can know that rib cartilage rhinoplasty is a relatively complex rhinoplasty surgery. Since modern beauty-orthopedic focuses on minimizing the wounds, and take micro-injury as future trend, it seems that rhinoplasty surgery with rib cartilage is on the opposed direction. However, this surgery skill is still valuable for the self body's utilization with the following conditions:

1. Patients of multiple rhinoplasty surgery and are having apex collapsing problem.
2. Patients who has been implanted with artificial materials but failed for affection or rejection issues.
3. Patients suffering for congenital nasal defect or severe upturned nose.
4. Patients of severe injury caused nasal deformation that is unable to be fixed with artificial materials.
5. Patients who refuse any artificial implants.

According to the author's observation, it would be better to use artificial materials which are easy to get in cases of simple rhinoplasty surgery that would be more convenient for further maintenance and stable effect duration. Plus, it is a lot easier to recover if one decides to remove the stuff. As for the issue that whether a patient should take out a piece of rib cartilages to perform rhinoplasty surgery, we leave it to the wise customers to choose from.
