Midlife Crisis of Young Ladies – Atrophy and Sagging of Breast

Breast is not only an aesthetic organ but also responsible for function such as breastfeeding. Therefore, it can be interpreted as the most important secondary sex organ in women. However, some of you may feel that your breasts have experienced changes after turning thirty. The most common concern is the loss of breast volume with unknown reason, especially after breast swelling from pregnancy, breastfeeding and breastfeeding withdrawal. Besides losing its volume, the breasts may even become atrophy and saggy. With the many years of outpatient observation, the author has discovered that more than 40% of patients with mammaplasty have returned to seek for consultation due to the changes of their breasts. This shows that most young ladies are very concern and cannot accept this type of changes to their body shape.

How does breast atrophy happen? Firstly, we have to understand the physiology of breast. Breast is comprised of fat tissues, mammary ducts and mammary glands. Besides, there are more fat tissues than mammary gland, accounting for approximately 60% of the total breast volume. Therefore, breast ptosis may occur when skin and ligament begin to lose its elasticity due to ageing or diseases. Moreover, weight loss and withdrawal of breastfeeding may cause the loss of fat and mammary glands which eventually lead to breast atrophy. Most patients only realize the change of breast shape during this stage and only desperately seek for solution by then. However, most of the information and supplements accessible by the public are often ineffective or even harmful. Eventually, these patients can only seek plastic surgery to restore their body shape.

On the other hand, the principle of improving breast atrophy is indeed very simple. 

Presently, there are a few options of treatment which employ surgical approach to restore the breast shape or make it better than before.
1. Autologous fat or macro-molecules hyaluronic acid injections:
Due to the limited doses in this type of breast augmentation, this treatment can only help to increase one cup size. Therefore, this is more suitable for those who wish to restore the original size of their breasts or receive partial modifications (such as filling the depressed upper breasts). There is no recovery period and no massage is necessary for this type of treatment. Besides, touch of the breasts is identical to natural breast. The shortcomings of this treatment include the expensive fees, necessary of repeat injection and possible mastitis or lumps.
 
2. Endoscopic mammaplasty:
Regular mammaplasty can be adopted for the purpose of breast augmentation to improve mild to moderate breast atrophy. Currently, the options of breast implants include saline implant and silicone gel implant. For those who wish to enlarge their breasts besides improving saggy breasts, the author would suggest mammaplasty as the fastest and effective approach. However, more pain is associated to surgical approach when compared to other treatments. In addition, post-operative massage for half year is required to maintain the breast softness and natural results.
 
3. Combined maxtopexy (breast lift) and mammaplasty:
This type of surgery is more suitable for patients with naturally oversized breasts leading breast ptosis. Generally, this type of patients has natural breast size of C – D cup. Once the skin becomes floppy, breast ptosis would occur. However, insertion of implants by itself is insufficient to effectively improve the condition. Therefore, additional breast lift surgery is required to achieve satisfying results. Drawbacks for this type of treatment include the possible of scars on the areola or breast area and higher surgery expenses than mammaplasty.
 
Breast ptosis is medically defined into several categories (as shown in figure 4). Readers can self-diagnose their condition by referring to the diagrams. Generally, grade 1 to 3 can be corrected by mammaplasty with insertion of implants. However, patients at grade 4 or above may require both maxtopexy and breast reduction surgery. Accordingly, different conditions may require different surgical approaches for instant improvement results.
