Revision mammaplasty
 
1. Is it necessary to replace the water bags if undergoing a revision mammaplasty? Can the original breast prosthesis be kept?
   

In principle, if the surgery is performed either through the bottom of the areola or the bottom of the breast, but not through the axillary incision, then the original breast prosthesis can be kept as consumers’ request. However, once the breast prothesis has been removed, the infection risk would be rising if the prothesis were being re-implanted. Furthermore, the durability of the breast prothesis is reduced after several years; especially, the re-installed silicone implants have to be very careful in future. For safety’s sake, I would suggest you replace new breast implants unless you have a tight budget.

2. Is the recurrence of capsule unlikely to occur after the removal procedure? Won’t consular contracture occur in future?
   
No. When our bodies detect any smooth foreign matter, the body tissue will automatically produce a capsule to surround it. That is the formation of a capsule and the body’s natural reaction. The removal surgery of the capsule removes the hard and thick capsule from your body to return you a soft and bigger space. As a result, it won’t easily occur capsular contracture. On the other hand, the capsular incision operation only looses the tissue without removing the capsule from the body. Therefore, there is a good possibility of the capsule recurrent and thicken. In any case, the patients must frequently massage their breasts after receiving a revision mammaplasty.
3. Would it be better to apply silicone gel-filled implants for revision mammaplasty?
   

It isn’t necessarily so. It must also be considered depending on individual’s demand. It would be better to apply silicon gel-filled implants if the patients who received the first surgery do care about the fold appearance of the water bags or the unnatural feeling when touched them. Moreover, if you have extremely thin skin tissue or a skinny figure, silicone implants can avoid any touch defect in future. If you were in a good condition, it would not be different between saline implants or silicone implants.

4. Why is the surgery fee for revision mammaplasty much higher? Is there any way to save some money?
   

The fee depends on the surgery length because of the increasing anesthesia expense consists with the surgery time. In addition, the fee would be higher when patients apply silicone gel-filled implants. If you are going to save some money, you can consider not changing your breast implants or just correct one side of the breast with more apparent and serious problems. However, I believe that the doctor would assess the difficulty and time of your surgery to determine the surgery fee. I suggest you consult with your doctor in advance.

5. Are the capsular removal and the breast prosthesis implantation carried out at the same time? Is it possibility to perform these two surgeries by stages?
   

The procedures of capsular removal and breast prosthesis implantation are performed at the same time for majority of revision mammaplasty. But an excepted condition occurs when patients have capsular contracture, the implants must be placed under the muscle instead of the original site which under the fascia. Because of the breast skin has already been propped up by the breast prosthesis, (the fascia is under the skin), it becomes loose. If we removed the implants to other position, the appearance of the breasts would be like wrinkled and sagging bags. I always suggest our patients a six months recovery time after removing the breast prosthesises in order to facilitate skin and fascia go back to the original positions. Hence, that we re-install new breast prosthesises after an appropriate recovery will achieve satisfactory results.
