Secondary rhinoplasty

The outcome of rhinoplasty varies greatly due to the different condition of the person’s original nose. Furthermore, factors such as surgery method, choosing nose mould and the surgeon’s sense of beauty will often cause consumers to have a subjective view thus consumer may be unsatisfied after the surgery. Even after some period of time, complications will occur after rhinoplasty. For example, nasal dorsum starts to become crooked, nose head becomes red, hurts and so on. If these symptoms continue to appear, then there is a need to consider a secondary rhinoplasty to correct it.

If you have circumstances listed below, you can consider under going another rhinoplasty in order to see improvements.

1. Subjectively judging the look of the nose is unnatural or inaesthetic
2. The nose is crooked or movable due to an external force
3. redness in the nose head or aching nose
4. Adventitious inflammation of nasal dorsum or symptoms such as rejection of the nose mould

There are many categories in Korean style rhinoplasty, which can completely resolve the problems caused by previous rhinoplasty.

1. Open incision
Enter from the incision of nasal columella, separate the soft skin tissues and the cartilage below it. As a result, the skin of the nose head can completely relax. This will prevent problems such as redness, become thin or even penetration of the nose mould, which are due to the nose head being too stiff.


2. Removal of the capsule
If previously the implant material is silicon nasal bone, then a layer of uneven thickness capsule cladding must have been created. This capsule must be cleaned completely to resolve pre-surgery contraction of the nose or discomforts. In addition, the nasal dorsum and nose head will also become softer and thinner due to withdrawal of the capsule. This is helpful in the reduction of the nose head and re-sculpturing.

3. Cushioning of nasal dorsum implant
If patient reject silicon nose mould then change to using Goretex, which can cohere to tissues, in secondary rhinoplasty to solve problems like discomforts occurred previously.

4. Cushioning of the cartilage of the nose head
The effect of using autologus nasal septal cartilage to create a nose head that is mignon and pointed is much better than using ear cartilages. Cartilages can be used as a cushion between the nose head skin and artificial material to ensure the safety of the nose head.

5. Thinning of the nose head and focusing of the cartilage on the two sides:

This key is to remove the capsule and fat tissues inside the nose head. This will improve problems of the nose head being too hard, too big or even “garlic nose” and so on.


6. Shrinking the nose wing or nostril
Let the triangle ration of the nose head to be more perfect. The surgery, which can be operated internally or externally, will be different depending on the person’s size and thickness of the nosewing
 

In secondary rhinoplasty, you can choose to change to the implant material to the new improved silicon nose mould or even more expensive materials such as Gortex. This two types of material is currently the main trend in rhinoplasty and they both have their advantages and disadvantages. However, if the patient has been previously infected by silicon nose mould, nose is inflamed or other complications then the author suggest to choose to use Goretex in the secondary rhinoplasty as it is compatabile with the body tissues in the long term and does not shack easily or relocate.

From subjective point of view, generally the author will suggest patients with sufficient certain nose head condition to choose to use artificial nose mould for the implantation. The outcome can be easily achieved. However if the nose head is too tight or the condition is not allowable then patient must choose to use Goretex to achieve the goal of the surgery which is beauty and safety.

The natural look of the nose depends on the condition of the person’s nose, possible occurrence of problems such as over tightened and red and swollen nose. If it is not placed below the periosteum, relocation or side effects may occur.
It must be inserted from the incision of nasal columella. After insertion, the height will gradually decrease as time pass, easy to relocate or be uneven, uneasy to take out later (half year later), need to cooperate with patient’s own cartilage and it is expensive
