Mandible reduction surgery

Having a wider mandible is a common phenomenon in the southern ethnic group. For example, the so called wide, round or square faces are all due to the result of a protruded mandible or its aggrandizement. As a result, women who like to pursuit for oval shaped face will do anything and think all types of methods to shrink their cheeks to allow the flow line of the face to look more smooth and perfect.

The mandible area is a complex structure, which is located from below the ear and extending to the side of the jaw. Its makeup most importantly is the mandible and the chewing muscle, which is closely attached above it. The increase in the thickness of both of these structures all have the possibility to cause the mandible to protrude. However, problems with the mandible it self accounts for 80% of the reason. In other words, the effect of patients, in high proportion, relying on injecting botulinus on the chewing muscle to let the face become skinner is limited. Therefore if wanting to seek obvious and forever lasting effect, then patients have to consider mandible reduction surgery.


There are three major types of Korean style mandible reduction surgeries. They are mandibular angle reduction surgery, mandible reduction surgery and mandibular grinding surgery with partially chewing muscle resection. Surgeons are required to plan and select the desired surgery method and area during the pre-surgery evaluation. There is also the possibility of using the combination of two surgical methods in the surgery in order to achieve the best effects depending on the protruding degree of the mandible.

The traditional mandible reduction surgery requires cutting open an incision on the surface of the skin, separating layer by layer from the outside to the inside. This will not only leave very nasty scars, it might also damage the nerves or blood vessels of the face. What our clinic uses is the Korean style reduction malarplasty. The surgery is conducted under general anesthesia. Firstly, open an approximately 3cm incision on the lower alveolus mucous near the mouth cavity. Along under the periosteum, separate it apart until the whole mandibular angle and bone can all be seen clearly. After that, mark the desired rage that will be resected and was previously measured on the X-ray film. Then the instruments for the Korean style reduction malarplasty can be used to proceed in the reduction steps. Depending on the surgical range and difference in methods, the whole procedure will mostly be completed in 1~ 1.5 hours. After examining that the two sides are symmetrical and correct, lastly, insert the two drainage tubes in the mouth. Then, after the incision has been stitched up, the surgery can be completed.

After the surgery, the face needs to be pressured for 24 hours by Elastoplasts to prevent post surgery swelling or hematoma. As for the purpose of inserting drainage tubes in the mouth is to direct the possible accumulated blood water after the bone reduction surgery and let it drain outside the body. Depending on the amount of the blood water, the drainage will be removed on the day before returning home or on the next day at the latest. Three days after the surgery, and after the swelling has gradually cleared away, patients are required to practice exercising the mouth. One week after the surgery, patients are required to eat liquid foods, and eating conditions can return back to normal after two weeks.
